Liver-related pain is usually felt in the upper right side of the abdomen , just under the ribs, and it can sometimes spread to the right shoulder or upper back.

What it can feel like

  • A dull ache, pressure, or soreness under the right rib cage.
  • A sense of fullness or bloating in the upper right belly.
  • Sometimes pain is felt in the back or right shoulder rather than directly over the liver.

Important note

The liver itself does not usually “hurt” in the way skin or muscles do; pain often comes from swelling or irritation around it, or from nearby organs that can feel similar. So right-upper-abdominal pain is not always from the liver, even if it seems like it could be.
