What 1095-A Is For

Form 1095-A is the tax form that reports your Marketplace health coverage and any advance premium tax credits you received, so you can complete Form 8962 with your federal return.

Where To Get Your 1095-A

If you used the federal Marketplace (HealthCare.gov):

  • Log in to your HealthCare.gov Marketplace account.
  • Choose your application for the correct tax year (for example, your 2025 application when filing your 2025 return).
  • In the menu, select “Tax Forms,” then under “Your Form 1095-A” click “Download PDF.”

If you used a state marketplace (like Covered California, NY State of Health, MNsure, etc.):

  • Sign in to your state Marketplace account and look for a “Tax Forms,” “Documents,” or “Notifications” section to download Form 1095-A.
  • Many state exchanges also mail a paper copy to the address on file each January or by early–mid February.

What If You Didn’t Receive It?

If you don’t see Form 1095-A online or never got it in the mail:

  • Make sure you are viewing the correct year’s application and tax forms.
  • If it’s still missing or looks wrong, contact the Marketplace Call Center (HealthCare.gov or your state exchange) and ask them to issue or correct your 1095-A.
  • The Marketplace, not your insurer, is responsible for sending a copy to both you and the IRS.
